A dependable, compassionate presence you can trust.
The best way to provide the highest quality level of care requires an initial meeting where family and/or client meet with a visiting CareLNK coordinator. • Functional assessment; physical needs for the care of the recipient such as grooming, mobility, dietary needs/restrictions, meal preparations.
  • The family support system and importance of continuous family presence always.

  • Goals - Measurable and expectations with the client/patient and their family.
